General Summary: An exempt position providing physical therapy evaluations, modalities and treatment in accordance with established clinic and departmental policies.
Position: Full-Time
Hours: M-F, 8a-5p
Essential Job Responsibilities:
Performs patient evaluations and establishes plan of care.
Assesses the effectiveness of treatment and modifies treatment to achieve goals.
Provides care in accordance with physician’s orders, protocols, and established plan of care.
Maintains proper documentation and records for patient charts.
Assesses educational needs of the patient, family, caregiver or significant other and provides education to meet those needs.
Plans for discharge.
Communicates with patient, family, caregivers, significant other and members of the healthcare team to promote maximum benefit of care.
May be asked to assist at satellite Therapy Clinics and will receive mileage reimbursements.
Education: Licensed Physical Therapist with the State of Tennessee.
