Appointments
We see patients by appointment only. You can make your appointment over the telephone or request your appointment online through our appointment center. Please arrive 15 minutes before the scheduled appointment time to allow for sufficient time to complete all necessary information. To complete the patient forms before the scheduled visit, click here or refer to the forms under Patient Resources. Be sure to bring your insurance card and photo identification at the time of your scheduled appointment.
Appointments should be cancelled within 24 hours to allow other patients an opportunity to be seen.
Referrals
If your insurance company requires a referral from your primary care physician, please provide the clinic with all necessary referral information and documents. You must have this referral prior to seeing the physician.
